Mature industrial sectors (e.g., aviation) collect their real world failures in incident databases to inform safety improvements. Intelligent systems currently cause harms without a collective memory of failings. As result, companies repeatedly make the same mistakes design, development, and deployment intelligent systems. A collection system experienced (i.e., incidents) is needed ensure benefit people society. The AI Incident Database an initiated by industrial/non-profit cooperative enable avoidance mitigation. database supports variety research development use cases with faceted full text search on more than 1,000 reports archived date.
